Terms & Conditions - the basics

1. Services Provided

Revival of Hope Wellness provides professional mobile massage therapy services, including but not limited to:

• Basic Facial

• Swedish Massage

• Deep Tissue Massage

• Targeted Massage

• Prenatal Massage (with physician approval)

All services are provided by a licensed massage therapist and are for therapeutic and relaxation purposes only.

2. Appointment Booking

Appointments may be scheduled online, by phone, or via text confirmation.

• A valid credit/debit card may be required to reserve your appointment.

• Same-day appointments are subject to availability.

• Clients must be 18 years or older (minors require written parental/guardian consent).

3. Cancellation & Rescheduling Policy

We value your time and ask that you respect ours.

• 24-hour notice is required to cancel or reschedule an appointment.

• Cancellations made less than 24 hours before the appointment may be charged 50% of the service fee.

• Same-day cancellations or no-shows may be charged 100% of the service fee.

• If the therapist arrives and the client is unavailable, the appointment will be considered a no-show.

4. Arrival & Setup Requirements

• As a mobile service, clients must:

• Provide a clean, safe, and smoke-free environment.

• Allow adequate space (approximately 6x10 feet) for the massage table setup.

• Ensure pets and children are secured during the session.

• If the environment is deemed unsafe or inappropriate, the therapist reserves the right to refuse service and full payment may still apply.

5. Health & Medical Disclosure

• Clients must complete a health intake form and disclose all medical conditions, injuries, and medications prior to the session.

• Massage therapy is not a substitute for medical care. Clients with the following must provide physician clearance:

• Recent surgeries

• Pregnancy (especially high-risk)

• Cardiovascular conditions

• Severe osteoporosis

• Blood clotting disorders

• Revival of Hope Wellness reserves the right to refuse service if massage is contraindicated.

6. Professional Boundaries

• Revival of Hope Wellness maintains a strict professional standard.

• Massage therapy is strictly therapeutic and non-sexual.

• Any inappropriate behavior, comments, or advances will result in immediate termination of the session.

• Full payment will be required if a session is terminated due to client misconduct.

7. Payment Policy

• Payment is due at the time of service.

• Accepted forms of payment: [Insert forms – e.g., cash, credit card, etc.]

• Gratuity is appreciated but not required.

• Prices are subject to change without notice.

8. Liability Waiver

• By booking an appointment, you acknowledge that:

• You are voluntarily participating in massage therapy.

• You assume responsibility for any risks associated with treatment.

• Revival of Hope Wellness is not liable for injuries resulting from undisclosed medical conditions.

9. Right to Refuse Service

• Revival of Hope Wellness reserves the right to refuse or discontinue service at any time for reasons including but not limited to:

• Inappropriate behavior

• Unsafe working conditions

• Health concerns that contraindicate massage

10. Privacy Policy

• Client information is kept confidential and will not be shared without written consent, except as required by law.

11. Changes to Terms

• Revival of Hope Wellness reserves the right to update these Terms & Conditions at any time. Continued booking of services constitutes acceptance of any changes.
